Submitting your proof of vaccination will provide you with your QR code so you can access public areas that require a vaccine passport. Check the City of Hamilton website to stay updated on vaccine requirements in public spaces.
Submitting to Ontario’s vaccine system
- You must complete a request form and submit a copy of your vaccine proof to the Hamilton Public Health department via mail, fax or in person. This will take around seven days to process. Learn more about this process on the City of Hamilton website, under “Proof of Vaccination Outside Ontario.”
- Once Hamilton Public Health processes your request, you can get your QR code on Ontario’s COVID-19 website.
